Team Presents Results of Quality Improvement Program

Members of the City of Lakes Family Health Team were recently invited to make a presentation at the Quality Improvement and Innovation Partnership (QIIP) conference in Toronto on January 27, 2010.
The members of the QIIP team have led a quality improvement program focused on chronic disease management, preventative medicine and operational improvements. The team has addressed health issues such as diabetes management and colorectal screening. It has also implemented an Open Access initiative in which most patients are able to make same day appointments with their physician- Dr. Ceaser.
The improvements experienced in this initiative will be disseminated throughout the family health team in the future.
