Our team recently unveiled a new Patient Charter which identifies the rights and responsibilities that we expect of our patients and ourselves. The charter is posted in all of our reception areas on large information boards. These boards also provide information about family health teams, frequently asked questions and the members of our team.
The Patient Charter states the following:
Patient Rights
·To be treated in a safe and secure environment
·To be treated in a kind and respectful way
·To participate in the care that’s provided to you
·To ask questions of the people providing your care
·To refuse to receive care you don’t want
·To have your personal health information remain private
·To be treated with respect and dignity
·To a scent-free environment
·To receive reasonable notification when an appointment has to be re-scheduled (preferably 24 hours)
Patient Responsibilities
·To treat our staff with courtesy and respect
·To be polite and respect other patients
·To take responsibility for your health
·To give accurate information to help plan your care
·To take care of your personal belongings
·To accept responsibility for the choices and decisions you make about your care
·To bring your health card to every appointment
·To provide reasonable notification to the clinic when cancelling or re-scheduling your appointment (preferably 24 hours)
