Hours:          37.5 hrs per week
Status:         Temporary, Full-Time (Maternity Leave)
Remuneration: salary range- $66,000 - $78,498

Start Date: April 2019

Job Purpose:

The City of Lakes Family Health Team (CoLFHT) is funded to hire a Quality Improvement Decision Support Specialist (QIDSS). This position will serve a partnership consisting of nine (9) Family Health Teams (FHTs) located in northeastern Ontario. Members include Espanola, Parry Sound, Powassan, West Nipissing, Northeast Manitoulin, Assiginack, Manitoulin Central, Huron Shores and Sudbury. The partnership utilizes the same electronic medical record (EMR) system - Telus Health PS Suite. The QIDSS will support the collective efforts of the northeastern partnership to develop and implement annual quality improvement plans (QIPs) and manage EMR data. This will require some versatility and flexibility in order to meet the diverse needs of the participating FHT partners.

Key Areas of Responsibility and Duties:

Data Management

 ·        Be a subject matter expert in data standardization and lead the analysis of data quality issues, using problem-solving methodologies to recommend corrective and preventative action
·        Provide counselling and analytical support regarding the collection, interpretation, analysis and presentation of data to help teams ensure that clinical information management practices support best practice standards.
·        Design, test and deploy tools and processes for extracting and managing performance data
·        Identify gaps in data availability, and define and implement solutions to close the gaps
·        Conduct analysis and develop reports and presentations in support of boards and administrative leadership
·        Provide support in the development and annual review/updates of the Annual Operating Plans, particularly around program planning, indicators, measures, outcomes evaluation and enhancements
·        Develop and maintain EMR tools used for improving data integrity or quality improvement
·        Maintain and develop other forms of data collection through patient engagement tools such as tablets or online forms

Quality Improvement

·        Collaborate closely with multi-disciplinary teams engaged in quality improvement helping to identify performance measurement information and data quality needs
·        Support the work of boards and quality improvement committees in the development and implementation of annual quality improvement plans
·        Act as a change agent and coach to inter-disciplinary teams on data management and performance measurement
·        Support the development, implementation and understanding of regular reports such as Data to Decision (D2D), and HQO Primary Care Practice Reports

Communication and Coordination

·        Conduct an initial in person evaluation of each FHT to assess QI needs and priorities; 
·        Meet with each FHT in the partnership twice annually or as required;
·        Support and attend quarterly partnership meetings;
·        Participate and support the development/review of the Annual Workplan for the QIDSS position;
·        Regularly communicate with partnership members via telephone, email and videoconference; 
·        Assist with documentation and sharing of best practices and success stories of data management and quality improvement
·        Coordinate and communicate with other QIDSS’s and provincial-level QIDSS staff on data management, community-based needs and quality improvement;
·        Stay current and maintain regularly contact with software and database providers such as Telus PS Suite, Ocean by Cognisant MD, Ontario MD, and North Eastern Ontario Network
·        Stay current with the latest quality improvement trends and tools in healthcare;
Participate in MOHLTC, HQO, NELHIN and AFHTO initiatives and training opportunities regarding data management and quality improvement


·        University degree in health administration and planning, health informatics, information management, public health or equivalent experience and education;
·        Understanding of primary care and multidisciplinary team based approaches to health care delivery;
·        Experience in health sector data management, working with electronic medical records and quality improvement initiatives;
·        Experience in developing and generating user-friendly reports;
·        Essential skills include problem solving, time and project management, interpersonal skills, analytical reasoning and communications; 
·        Ability to manage multiple demands and interpret and evaluate data;
·        Demonstrated ability to provide individual and group training; and,
·        Ability to travel by car to remote locations.
