CCAC Care Coordinators

The Northeast Community Care Access Centre (CCAC) has teamed up with the City of Lakes FHT to improve patient care. This collaboration brings three fulltime CCAC Care Coordinators onsite at the FHT clinics to serve homecare clients who are our patients. The purpose of the program is to improve homecare for CoLFHT patients by increasing the level of collaboration and communication between CCAC case managers and CoLFHT clinicians. The program began with a pilot program in 2012 and has proven to be very successful. Our patients and their caregivers find it easier to access homecare services and our clinical providers are able to effectively monitor the quality of homecare being provided to their patients. Much of this quality care is attributed to the dedication and commitment of our CCAC Care Coordinators- Roxanne McGrath, Lise Waltenbury and Lani Radley.
