You can now book non-urgent appointments with your physician or a nurse practitioner.  To do so just grab your health card, visit our website at www.yourfht.com and select the “Book Your Appointment” button. 

Need to send us a message?

You can now send us a non-urgent, secure message directly through our website.  Whether it is to cancel your appointment, update your medical records, or submit a form – all of it can be done with just a few clicks!  To send us an online message safely and securely, visit our website at www.yourfht.com and select “Send us a Message” button.  All you need is your health card!
