Interdisciplinary TeamThe City of Lakes Family Health Team is an interdisciplinary team of local family physicians and health care professionals working collaboratively to provide comprehensive primary health care services to their patients. Our team includes family physicians, nurse practitioners, registered nurses, a pharmacist, a dietician and a social worker.
Our clinics are networked through the use of electronic medical records, Telus Health PS Suite, instead of traditional paper charts. These records are stored and protected in our computer system. Ultimately, this means better communication between all members of the Family Health Team. It will also allow your medical records to be easily accessible, and securely shared with specialists, hospitals and other health care providers as needed. No matter where you are or who you are seeing, that health care provider will have up-to-date information concerning your medical history, current medications and recent test results. Our clinics are connected by a state-of-the-art broadband data network.
Electronic Medical Records
We are a member of the Ontario Telemedicine Network (OTN). Telemedicine is the delivery of health-related services and information using telecommunication technologies. All of our clinics are equipped with a telemedicine suite. We normally schedule over 1,000 clinical appointments annually for our patients.

We are committed to seamless care for our patients. That means that all doctors on our team have hospital privileges; therefore, they can visit you if you have been admitted to Health Sciences North.
Our clinical team provides clinical placements for medical students, family medicine residents, nurse practitioner students and other learners from health-related disciplines. We believe that learners who experience clinical opportunities in an interdisciplinary setting are more likely to join practices such as our family health team once they have graduated.

Urgent care after hours clinics
We will also provide limited access to a team physician or nurse practitioner during evenings for urgent care. You are required to make an appointment for this after-hour service. These appointments are designed to provide patients with timely medical care to help deal with issues such as infections (e.g. ear infections) or when a current medical condition changes (e.g. asthma). Please do not use the afterhours clinics for non-urgent services such as prescription renewal. Please note that you may be seeing any one of our team physicians or nurse practitioners since all providers will be sharing the responsibility of providing after hours care. The after hour clinics are offered at one of our clinics Monday-Friday from 5:00pm - 8:00pm.
This service is only available to patients who are rostered to a family health team. The Telephone Health Advisory Service is a Ministry of Health and Long Term Care initiative aimed at improving access to primary health care services for Ontario residents enrolled with a Family Health Team. When you call the Telephone Health Advisory Service you will be connected to a nurse who will verify that you are enrolled with the Family Health Team. Based on the information you provide the nurse will provide health care advice or advise you to see your doctor or nurse practitioner within a few days.

We have developed partnerships with several local healthcare agencies including Health Sciences North, the North East Specialized Geriatric Services and the Northeast Local Health Integration Network, among others.

The City of Lakes Family Health Team is a non-profit corporation that is governed by a community-based Board of Directors. Board members include physicians, health care professionals and patients of the City of Lakes FHT.
