The Canadian Mental Health Association (CMHA) has taken a lead at establishing a Health Links partnership in Greater Sudbury and has recently begun discussions with the City of Lakes FHT and other community partners.
Health Links are supported by the MOHLTC and are intended to connect teams of professionals that share information and coordinate care for individuals with specific complex care needs—typically the top 5% of health care users. These patients use services that account for approximately two-thirds of Ontario’s health care dollars.
Better coordination of care for these patients will contribute to better patient experiences, outcomes as well as significant system savings.
The proposed Health Links for Greater Sudbury would respond to the needs of individuals impacted by mental illness, addiction and/or developmental disability. These individuals have complex conditions, requiring support from multiple care and service providers.
